Where: Edwards Cafeteria, on 6th Street
Time: 3:00-6:30PM
Who: Any child needing immunization for school
What must be brought with the student:
The most up-to date shot record so the nurses may assess shots needed.
A parent/guardian must accompany students under the age of 15 years.
If student is covered by insurance, a card must be presented for billing purposes.
Each immunization will cost $21.96.Payment arrangements may be made.
NO STUDENT WILL BE TURNED AWAY FOR INABILITY TO PAY
