Yamhill County Public Health Department is holding an immunization Clinic for the Newberg School District.
When: Tuesday, November 2, 2010
Where: Edwards Cateteria, on 6th Street
Time: 3:00-6:30pm
Who: Any child needing immunizations for school
What must be brought with the student:

The most up-to-date shot record so the nurses may assess shots needed.

A parent/guardian must accompany students under the age of 15 years.

If student is covered by insurance, a card must be presented fo billing purposes.

Each immunization will cost $15.19. Payment arrangements may be made.

NO STUDENT WILL BE TURNED AWAY FOR INABILITY TO PAY
