Is your 7th grader one of our seventh grade students that does not have an up-to-date tdap vaccination (definition of up-to-date: within the last 5 years or after 10th birthday)? Seventh graders without this vaccination will be excluded from school in mid-February. When your student receives the tdap vaccination, please bring the record to our Counseling Office.
Here's an opportunity to get that vaccination!
When: Tuesday, November 15, 2011
Where: Edwards Cafeteria, on 6th Street
Time: 3:00-6:30PM
Who: Any child needing immunization for school
What must be brought with the student:
*The most up-to date shot record so the nurses may assess shots needed.
*A parent/guardian must accompany students under the age of 15 years.
*If student is covered by insurance, a card must be presented for billing purposes.
*Each immunization will cost $15.19. Payment arrangements may be made.
*NO STUDENT WILL BE TURNED AWAY FOR INABILITY TO PAY
