Newberg School District and Providence Heart and Vascular are proud to announce an exciting opportunity for all students age 12 and older. Providence Play Smart Program is offering free heart screenings on April 17th at the Newberg High School Gym. This screening includes Free EKG's, blood pressure reading, height and weight check and echocardiogram if needed.
To participate please obtain a consent form from any PE teacher, the school office or download the form below. You do not have to be in a PE class or in a sport to participate. Bring the consent form with you on the day of the event.
Please return the Field Trip Permission slip to the front office by 3:00 on Friday, April 13th to reserve your student a spot.
