Does Your Student Athlete Need a Sports Physical?

Sports Physicals are being offered for winter and spring athletes on Tuesday, November 1st from 6:00 – 8:30 PM in the Newberg High School Main Office.

The cost is $25. To pre-register please call 503-538-9119.

Every student-athlete must have a current, State of Oregon physical form on file prior to the fist day of practice. Physical exams are valid for two years.
A parent or guardian must be present and sign authorization forms for children under the age of 15. Students should wear a loose T-shirt and shorts.
