Free Sports Physicals
Providence Newberg Medical Center will be offering free sports physical for students in 3rd grade and up on Thursday, July 14 from 6pm - 9pm.

The Sports Physical must be completed before the 1st day of practice and is valid for 2 years.
Wear a loose T-shirt and shorts.
A parent or guardian must be present and sign authorization forms for children under the age of 15.
