Depending on where your student is coming from we need the information below.

COA Application — Annually (for grades K-5)

6-12 Application coming soon!

Newberg SD Student Enrollment Packet (one-time, if student has not been enrolled in last 60 days, not incl summer break)

Once we receive the above you will receive the link for curriculum selection

Immunization records — Need current on file or opt out form 

Copy of Birth Certificate
