Newberg schools require a sports physical for your child to participate in an athletic program. Providence Rehabilitation Services is stepping in to offer FREE sports physicals Thursday, July 19, 6 p.m. - 9 p.m. at Providence Medical Group – Newberg (first floor), 1003 Providence Drive, Newberg.

A parent or guardian must be present and sign an authorization form for children under age 15. Student should wear a loose T-shirt and shorts. Questions? Call 503-574-6595
