Home COVID Testing Program
Hello families!
We at Rize Laboratory (formerly WVT Laboratory) are looking forward to supporting your students again this year with access to PCR COVID testing. As always, this program is completely optional and is flexible in the way you choose to use it. This year, there is a significant change in how kits get distributed to families. They will now be shipped directly to your homes.
Step 1: Enroll your student(s) by filling out the 2022-2023 consent form here. Minors 15 years of age or older may self consent to COVID-19 testing as ordered by the Oregon Health Authority under ORS 109.640(2)(a).
Step 2: Ensure that each student that has been consented into the 2022-2023 program has a LabDash account (you do not need to create a new LabDash account if you have an existing one). You can set up minors as dependents on your parent/guardian account. This step must be completed before we can ship test kits to you.
Step 3: Please allow 1-2 weeks from enrollment in the program to receive your first shipment of test kits. Each student enrolled will receive an initial 4 test kits with barcodes that are specifically assigned to that student, so please ensure that if you have multiple students enrolled that you're using kits that were shipped to them under their name. Test kits will still need to be collected in LabDash each week with name and date of birth written on the tube prior to turning in the test kit. Unlabeled and/or unlinked (in LabDash) test kits cannot be processed. Once the 4th test kit has been submitted to our lab, a new batch of test kits will be triggered to ship out to you automatically.
Step 4: Completed test kits can be returned to the school office Tuesday mornings before 10am, for our couriers to pick up, or 7 days a week, 9am - 5pm, at our testing site in downtown Corvallis: 366 SW Washington Ave.

Remember, these tests are paid for through programming in partnership with Oregon Health Authority and are intended for enrolled K-12 student use only. Testing sites/appointments for anyone not in the K-12 category can be found on our website: www.rizelab.com.
These are PCR tests that can be used to collect your sample at home, but must be processed by our laboratory; they are not rapid tests. Once your sample has been received by our lab, the expected turnaround time is 24-36 hours. Your school may have rapid test kits available for use.
While the COVID-19 surveillance program is designed to be used weekly, you can choose to use the test kits for your student(s) as desired. You can submit a test weekly, you can take weeks off, you can submit a test after an exposure, or if symptomatic. This is not an exhaustive list of how you can use the tests.
